You will need a certified copy of your Marriage Certificate and you cannot give a photocopy.  The Marriage Certificate needs to be from the Town Clerk rather than from the religious ceremony.  A Separation Agreement which is like a Divorce Agreement that must meet certain requirements.  There is a form called a “Joint Petition for Divorce” form, an Affidavit of Irretrievable Breakdown of the Marriage, which both parties sign, a Record of Absolute it is a R-408 form, a Military Affidavit which is saying if either of the spouses are in the military under the law or confirming that the parties are not, a Financial Statement that each party needs to sign and date. In addition, if you have children that are under the age of 18, you will need to do an Affidavit Disclosing Care or Custody proceedings and that is saying if there are any other custody proceedings regarding the minor children.  If there is something out of state or with DCF in Juvenile court and things of that nature, even if there is not you are saying there are none. You will have to do a Child Support Guidelines or a Worksheet that is a particular form that the courts fill out.

There are also lots of other forms that maybe needed; including Affidavits of Indigency; including Deviation forms for child support, you will need a check for $215.00, you will need a copy of your W2 form or 1099 from the prior year, but those are the basic forms that are usually needed for a Joint Petition.
