You should be able to argue to the judge that reason that you are asking for supervision including any specific concerns for either the safety of the parties or child(ren). The type of supervision that you are seeking whether that is at a supervised visitation center, hiring somebody to be out in the community, the name of the supervisor, how often and how long the visits should occur and if any restrictions in the location.

If there is going to be a review date to see for example to have supervised visits for 3 months and then get reviewed by the court and who is going to be responsible for the payment if that is part of the situation.
